Fiesta Food Employee Benefits Eligibility
Employees will be eligible to enroll for the Medical/Prescription, Dental, Vision, Life/AD&D, Supplemental Life/AD&D, Dependent Life, Voluntary – Accident, Hospital Indemnity, Critical Illness & Whole Life benefits if the employee is full-time, at least age 18, and have 90 days of continuous employment.
Also, if a part-time employee has averaged at least 30 work hours per week during an established measurement period, they will be offered the applicable benefit plans.
Fiesta Mart Employee Benefits – Explained
Basic Life Insurance:
Fiesta provides a basic life insurance benefit. The benefit amount is based on your eligible class and is payable to your designated beneficiary indicated during the enrollment process.
Your Insurance Benefit reduces at ages 70 and 75, respectively. Carrier is responsible for claims administration and approval of benefits.
Short Term Disability Income Replacement Insurance (STD):
STD provides partial income replacement protection if you are unable to work because of illness or injury.
Fiesta provides this benefit to benefits-eligible employees at no cost. Carrier is responsible for the claim administration and approval of benefits.
Long Term Disability:
Hourly and Salaried employees classified under Class I are provided access to a Long Term Disability benefit at the company discounted rate, but at an additional cost to the employee.
Traditional PPO Plan Highlights:
Preferred Provider Organization (PPO) – This Plan has a lower calendar year deductible and provides 75% coinsurance of covered plan expenses.
Member pays 25% at the point of service after deductible/copay is met, if applicable. However, if you use a Blue Distinction Center for knee, hip, or spine procedures, the Plan provides 80% co-insurance of covered plan expenses.
Dental Plan Benefits Highlights:
Dental coverage is offered by BlueCross BlueShield of Texas and utilizes the BlueCard Freedom Dental Advantage and DentaBlue Network.
Employees will have a choice of choosing a dentist in the BlueCareDental or DentaBlue network to lower out-of-pocket expenses. With more than 91,000 dentists throughout the US and 71,000 in Texas, BCBSTX offers you and your family flexibility in choosing a dentist.
Voluntary Supplemental Benefits:
Voluntary Supplemental Benefits are only available to employees and dependents that are enrolled in the group health plan. If you enroll in the following benefit plans, an additional weekly premium will be deducted out of your check.

While the typical doctor’s visit – from scheduling to prescription pick-up –takes an estimated four hours, an MDLIVE doctor’s visit takes approximately 16 minutes.
401(k) Plan:
Employees are eligible to enroll upon attainment of eligibility to participate in Fiesta’s 401(k) Retirement & Savings plan once the requirements are met:
• 21 years of age; and
• 90 days of continuous service with Fiesta
• 100% vested after 5 years of creditable service
• Catch-up, ROTH after-tax contributions also available
Vacation:
Full-time employees receive paid vacation time as outlined below (part-time employees also receive paid vacation time but in reduced amounts).
The amount of paid vacation time provided is based on the length of service and employment category on the most recent anniversary date. Vacation time runs from anniversary date to anniversary date and cannot be carried over from year to year.
