❑ Login to: FlexPlus Connect or download the Mobile App from the QR Code.
❑ Select FlexPlus Direct to Enrol.
❑ Select “Enrol” and follow the online instructions to confirm or make changes to your coverage.
❑ Verify your personal information and input your dependent information.
❑ Choose your benefits coverage for you and your dependents.
Enroll in your health, dental, LTD, and insurance coverage.
Enroll any eligible dependents.
Provide Coordination of Benefits (COB) for secondary health and dental coverage.
❑ Check your beneficiary designations and update them as needed. Designate beneficiaries for Life, Critical Illness, and AD&D Insurance. If you made changes:
Print the Beneficiary Designation Form (or request a copy by mail).
Sign and send back to the address provided on the form.
❑ Review your choices and select “Confirm” to lock in your choices.
❑ Print Confirmation Statement (or request a copy by mail) for your records.
Print and submit any other required documents (Evidence of Insurability, Beneficiary Forms).
